Complete your Middle School and High School Athletic Registration Online!

• You will create your FAMILY ACCOUNT during the initial registration process. To create the account, you will be asked to submit a username and password.  Creating a FAMILY ACCOUNT will give you access to your account information. There is a LOGIN button at the bottom of the page to access your account. 

• Please remember your username and password.  

• Once you have created a FAMILY ACCOUNT, you must then register each child individually within that family account. Registering a student needs to be done under the “Registration History” tab, NOT the “Family Member Info” tab.  For ease of filling out the forms, have your medical information (insurance card info, doctor's contact info, hospital info) handy.  Please note that not all information is required - only the boxes that have a * next to them.  Once you start the registration process you must complete at that time - no option to save and finish later. 

• Every athlete in grades 7-12 must complete all forms found on the Athletic Team Registration site before they will be allowed to participate in athletics this school year.  Registration must be completed for each student AND each new school year.

• The Athletic Physical Form AND Health History Form are REQUIRED prior to participation.  Click here for Physical Form and Health History Form.  Reminder - the physical is ANNUAL.  Be sure your doctor checks that option at the top of the form.

• Hard copies of these forms must be turned in to the main office at school prior to the first day of practice/tryouts.
